Job Description

The Clinical Care Coordinator plays a key role in supporting quality improvement initiatives, program planning, and interdisciplinary team development within Picture Butte Medical Clinic affiliated with the Chinook Primary Care Network (PCN). In addition to coordination responsibilities, the role includes the provision of direct patient care within the scope of nursing practice. Emphasis is placed on health promotion, illness prevention, chronic disease management, and patient education.

Key Responsibilities:

  • Coordinate and support clinic participation in Chinook PCN programs and initiatives.
  • Deliver basic nursing care to individuals, families, and groups.
  • Collaborate with interdisciplinary team members to provide comprehensive patient care.
  • Support program planning, evaluation, and quality improvement initiatives.
  • Promote and deliver health education and chronic disease prevention strategies.

Position Qualifications:

  • Registered Nurse in good standing with CRNA or LPN in Good standing with CLPNA with evidence of advanced competencies. Minimum of two years’ experience in primary care, community health, long-term care, or ambulatory care.
  • Cervical Cancer Screening/Pap Certification Course complete or willingness to take course and provide cervical cancer screening to patients
  • Strong written and verbal communication skills.
  • Preference for candidates with experience practicing to full scope.
  • Proficiency in health education for individuals and groups.
  • Demonstrated clinical excellence and sound nursing judgment.
  • Experience in interdisciplinary collaboration.
  • Knowledge and/or willingness to develop skills in program planning and evaluation.
  • Familiarity with chronic disease prevention, screening, and management.
  • Competency in computer applications including Microsoft Office, email, and electronic medical records (EMR).
  • Ability to maintain patient privacy and confidentiality.
  • Current CPR certification (additional certifications considered an asset).
