Job Description :
The Clinical Care Coordinator (CCC) plays a leadership role the Crowsnest Medical Clinic as a point of contact for the clinic’s participation in the Chinook Primary Care Network (CPCN). The CCC takes a lead role in program development and improvement in the clinic environment, including both operational and patient care teams, in partnership and guidance from the Clinic Manager and physician group. The CCC is supported by the CPCN Primary Health Care Integration Lead, Clinical and Quality Improvement Lead and the Evaluation team. The CCC has a strong understanding and commitment to quality improvement processes, program development, and management/integration of multiple projects including patient education and direct patient care. The CCC is not responsible to directly supervise or manage clinic staff.  

Position Summary: 
Working with the physicians and patient care teams, the CCC is responsible for ensuring the provision and performance of chronic disease programs in the clinic in accordance with Chinook Primary Care Network Standards. 

Roles and Responsibilities: 

  • ·          Investigate the availability of and facilitate access to local programs to support patients with chronic diseases in accordance with CPCN Standards
  • ·          Liaise with Alberta Health Services (AHS) to identify CDM  needs with the Crowsnest Pass
  • ·          Develop and implement CDM Programs to meet local needs through the Crowsnest Medical Clinic
  • ·          Ensure that clinical practice guidelines and protocols for CDM patients are in place and reviewed on an annual basis
  • ·          Ensure that all data related to CPCN quality improvement programs is accurate, recorded in a timely manner and reported based on the schedule set out by the CPCN
  • ·          Analyze quality improvement data and facilitate clinic improvement activities based on current results
  • ·          Develop and maintain a database of existing services within the community that support the patient as a whole; including social services, education, health, recreation and local employers.
  • ·          Establish key linkages with the community to support patients in actively managing their health in collaboration with the clinic care team.
  • ·          Provide education and support to newly diagnosed chronic disease patients, either one on one or in a group setting
  • ·          Identify the need for and coordinate the development of educational materials specific to CDM Programs offered by the clinic
  • ·          Develop orientation and training programs specific to the care team members
  • ·          Assess each care team members clinic skill and provide coaching, mentoring and training to ensure they are working to their full scope of practice
  • ·          Participate in the recruitment of care team members together with the clinic manager
  • ·          Annually or as scheduled,  participate in assessing care team members performance as it relates to their clinical duties
  • ·          Participate in,  and lead as appropriate, care team huddles or other meetings
  •            Perform other related duties, as required

Professional Certification / Designations
  • Registered Nurse/Baccalaureate Nursing degree
  • Basic Cardiac Life Support and Diabetic Teaching Certification is desirable.
Education / Experience
  • ·          Quality Improvement Training is an asset
  • ·          Chronic disease assessment, education and management experience preferred
  • ·          Commitment to, and knowledge of, primary care and community health
  • ·          Commitment to quality improvement and measurement with data driven decision making abilities
  • ·          Understanding of and commitment to working in an interdisciplinary team based primary care setting
  • ·          Strong leadership, organizational, facilitation and education skills

The Clinical Care Coordinator role combines quality improvement, program planning, and team development activities within a clinic environment to support the participation of the family practice clinic in the Chinook Primary Care Network.
The Clinical Care Coordinator will also provide basic health care within the scope of nursing practice for individuals, families, groups and communities. Working with other team members, the Primary Care Nurse provides comprehensive health care with an emphasis on healthy living, illness prevention, health education, chronic disease management, and clinical intervention.
 

Position Qualifications:
 
  • Baccalaureate degree preferred and current licensure with CARNA
  •  Minimum three years experience in a primary care, community health, LTC, and/or ambulatory care setting.  Preference will be given to candidates with experience working to full scope of practice.
  • Excellent written and verbal communication skills
  • Strong leadership, facilitation, and organizational skills
  • Demonstrates excellent clinical skills & knowledge
  • Proficiency OR a willingness to develop proficiency in use of computers including: MS Office, E-mail, MS Excel, PowerPoint, and electronic medical record
